Integrating primary care with mental health services, health promotion, and social supports, CHCs address the social determinants of health and advance equity in communities across Canada.<\/p>\n<p style=\"font-weight: 400;\">Over the past century, the CHC model has evolved in response to changing community needs. What began as localized efforts to serve underserved populations has grown into a network of organizations delivering interdisciplinary care, advancing health equity, and strengthening communities across the country.<\/p>\n<p style=\"font-weight: 400;\">The origins of Community Health Centres in Canada date back to 1926 with the founding of Mount Carmel Clinic in Winnipeg, Manitoba, recognized as the first CHC in Canada and still serving its community today. En int\u00e9grant les soins primaires aux services de sant\u00e9 mentale, \u00e0 la promotion de la sant\u00e9 et au soutien social, les CSC s&rsquo;attaquent aux d\u00e9terminants sociaux de la sant\u00e9 et favorisent l&rsquo;\u00e9quit\u00e9 dans les communaut\u00e9s partout au Canada.<\/p>\n<p>Au cours du dernier si\u00e8cle, le mod\u00e8le des CSC a \u00e9volu\u00e9 pour r\u00e9pondre aux besoins changeants des communaut\u00e9s. Ce qui a commenc\u00e9 comme des initiatives locales visant \u00e0 servir les populations mal desservies est devenu un r\u00e9seau d&rsquo;organismes offrant des soins interdisciplinaires, promouvant l&rsquo;\u00e9quit\u00e9 en sant\u00e9 et renfor\u00e7ant les communaut\u00e9s \u00e0 l&rsquo;\u00e9chelle du pays.<\/p>\n<p>Les origines des centres de sant\u00e9 communautaires au Canada remontent \u00e0 1926 avec la fondation de la Mount Carmel Clinic \u00e0 Winnipeg, au Manitoba, reconnue comme le premier CSC du Canada et qui sert toujours sa communaut\u00e9 aujourd&rsquo;hui.
