\"\"Hamilton Urban Core\u00a0Community Health Centre<\/a>\u00a0(Hamilton, ON),\u00a0offers\u00a0robust programs for the Black community they serve.\u00a0Initiatives such as the Black Men Wellness Group, Healing with African Music and Black Girls Talk\u00a0demonstrate\u00a0how\u00a0Black Social Prescribing\u00a0can strengthen connections and identity.\u00a0Their\u00a0Black Health Team offers culturally safe, community-based care for Black individuals grounded in\u00a0Africentric\u00a0lenses.\u00a0Coming\u00a0soon,\u00a0Obodo Maternal Care, a program for Black mothers and families\u00a0is\u00a0focused on breastfeeding\u00a0education and\u00a0decision-making support for\u00a0Black mothers while affirming their cultural beliefs, as well as Sickle Cell Disease Care to empower those living with this disease.<\/span>\u00a0<\/span><\/p>\n

\"\"Sexuality Education Resource Centre Manitoba<\/a>\u00a0(Winnipeg, MB) in partnership with the Winnipeg Regional Health Authority, developed\u00a0an Afro-Franco Outreach project\u00a0to enhance awareness of\u00a0sexual\u00a0health resources.\u00a0The\u00a0initiative\u00a0has hosted\u00a0multiple\u00a0sessions for youth,\u00a0teens 16-18 years\u00a0and adult groups, alongside\u00a0community awareness displays to share information and referrals.\u00a0This project\u2019s\u00a0objective\u00a0is to engage with Afro-Francophone newcomer groups.<\/span>\u00a0<\/span><\/p>\n

Rexdale\u00a0Community Health Centre<\/a>\"\"<\/a>\u00a0(Etobicoke, ON)\u00a0prioritizes\u00a0African,\u00a0Caribbean\u00a0and Black populations\u00a0within\u00a0their catchment area.\u00a0Rexdale\u00a0CHC\u00a0is piloting\u00a0the\u00a0TO Wards Peace\u00a0Project\u00a0child and youth program, which\u00a0aims to\u00a0interrupt\u00a0community violence and support those with lived experience around anti-black racism,\u00a0trauma\u00a0and violence.\u00a0They\u00a0also\u00a0offer\u00a0Black-led health equity programs that focus on mental health, chronic disease prevention and community wellness, youth empowerment and mentorship programs for Black youth facing barriers and Black women\u2019s health programs focused on maternal health.<\/span>\u00a0<\/span><\/p>\n

\"\"Somerset West Community Health Centre<\/a>\u00a0(Ottawa, ON)\u00a0offers several Black focused programs for the community in Ottawa\u00a0including\u00a0African, Caribbean\u00a0and Black HIV prevention, a Black Community Support Phone Line for service navigation, advocacy, and health coaching,\u00a0a Black Mental Health program\u00a0and\u00a0counselling services for the Black community as well as workshops and community events.<\/span>\u00a0<\/span><\/p>\n

\"\"Wellfort\u00a0Community Health Centre<\/a>\u00a0(Mississauga, ON)\u00a0is hosting\u00a0a Black History Month Event\u00a0on February 21st<\/span>\u00a0<\/span>with e<\/span>ngaging activities like Zumba\/Socacise, dance performances, spoken word poetry featuring Black artists, kids\u2019 activities, cartoonist, keynote\u00a0speakers\u00a0and cultural food.\u00a0This event has the goal of discussing\u00a0the future\u00a0of Black health innovation and the legacy of Black History Month.<\/span>\u00a0<\/span><\/p>\n

While February is dedicated to Black History Month, we are excited to see the work CHCs are doing every day, all year long, through Black social prescribing and culturally sensitive health care and support.
