She grabbed the bottle of Ibuprofen and took 400mg, silently hoping the pain would subside soon. Leaning back against the door, she muttered to herself, \u201cHow long will this go on? I can\u2019t live on pain meds forever. I have to see a dentist soon\u201d.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"color: #000000;\">Alisha remembered her last visit to a fee-for-service dentist, 5 years before. Since she didn\u2019t have dental insurance, she was given a bill of $1600; $800 for a root canal and an additional $800 for the crown. The total cost was\u00a0more than the combined cost of her rent, hydro and grocery bills for an entire month. \u201cWhat do I do?\u201d wondered Alisha, a client at<\/span> <a href=\"http:\/\/www.bramaleachc.ca\/\" target=\"_blank\" rel=\"noopener\">Bramalea CHC<\/a><span style=\"color: #000000;\">, one of five service locations\u00a0provided by<\/span> <a href=\"http:\/\/www.wellfort.ca\/\" target=\"_blank\" rel=\"noopener\">WellFort Community Health Services<\/a><span style=\"color: #000000;\">, a Communiy Health Centre located in Peel Region, Ontario.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"color: #000000;\">Alisha\u2019s story is true not only for a significant number of WellFort clients but also for many residents in the Region of Peel. Many barriers exist preventing a large proportion of our clients from attaining their full oral health and overall health potential; a huge one being the high cost of dental care. Based on the\u00a0recent<\/span>\u00a0<span style=\"color: #000000;\">2017 report<\/span>\u00a0<a href=\"https:\/\/www.peelregion.ca\/health\/resources\/pdf\/2017-oral-health-report-key-findings.pdf\" target=\"_blank\" rel=\"noopener\">Oral Health in Peel: Key Findings about Risk Factors and Oral Health Outcomes<\/a><span style=\"color: #000000;\">, 30% of local residents cited cost as a barrier to seeing a dentist.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"color: #000000;\">Dental care in Canada is operated mainly as a private, fee-for-service system. Public spending on oral health care is only <\/span><a href=\"http:\/\/cahs-acss.ca\/wp-content\/uploads\/2015\/07\/Access_to_Oral_Care_FINAL_REPORT_EN.pdf\" target=\"_blank\" rel=\"noopener\">6% of total spending on oral health care<\/a>. <span style=\"color: #000000;\">Canada<\/span>\u00a0<a href=\"http:\/\/cahs-acss.ca\/wp-content\/uploads\/2015\/07\/Access_to_Oral_Care_FINAL_REPORT_EN.pdf\" target=\"_blank\" rel=\"noopener\">ranks very low<\/a> <span style=\"color: #000000;\">among OECD (Organisation for Economic Co-operation and Development) countries for public spending on oral health care. Though there are patchworks of publicly-funded dental programs\/benefits across different provinces and territories for some vulnerable populations, these are inadequate and are a band aid solution at best. <\/span><span style=\"color: #000000;\">\u00a0<\/span><\/p>\n<p style=\"text-align: justify;\"><a href=\"http:\/\/www.karger.com\/Article\/PDF\/69846\">Evidence shows<\/a> <span style=\"color: #000000;\">that utilization of upstream approaches to improve population oral health result in\u00a0better long-term oral health outcomes and therefore offer the best solution.\u00a0<\/span><span style=\"color: #000000;\">A system where oral health care isn\u2019t a part of the universal health care system minimizes\u00a0the importance of oral health and puts the onus of oral health care on individuals.<\/span><\/p>\n<p style=\"text-align: justify;\"><a href=\"http:\/\/www.who.int\/oral_health\/media\/en\/orh_report03_en.pdf\">According to World Health Organization<\/a><span style=\"color: #000000;\">, oral health is a fundamental component of overall health and a determinant of quality of life.\u00a0 <a style=\"color: #000000;\" href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC88948\/\">Studies<\/a> have linked poor oral health with many adverse disease outcomes like<\/span> <a href=\"https:\/\/www.cda-adc.ca\/jcda\/vol-68\/issue-6\/340.pdf\">respiratory diseases<\/a>, <a href=\"http:\/\/www.hc-sc.gc.ca\/hl-vs\/iyh-vsv\/life-vie\/dent-eng.php\">heart diseases<\/a>, <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3073380\/\">mental health illnesses<\/a> <span style=\"color: #000000;\">amongst others. Oral health diseases<\/span>\u00a0<a href=\"http:\/\/cahs-acss.ca\/wp-content\/uploads\/2015\/07\/Access_to_Oral_Care_FINAL_REPORT_EN.pdf\">disproportionately affect vulnerable groups<\/a> <span style=\"color: #000000;\">who face barriers accessing oral health care. This includes under-resourced or low-income families, seniors, and indigenous populations among other groups.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"color: #000000;\">As an organization, WellFort is deeply committed to health equity. It is embedded in our vision and mission and is strongly reflected in our organization&rsquo;s <a href=\"http:\/\/www.wellfort.ca\/news-and-events\/whats-new\/2016\/12\/28\/wellforts-resolutions-for-2017\/\" target=\"_blank\" rel=\"noopener\">Resolutions for 2017<\/a>. Our commitment to health equity\u00a0has led us to focus significant energy on access to oral health care over the past number of years. <\/span><\/p>\n<div id=\"attachment_16781\" style=\"width: 310px\" class=\"wp-caption alignright\"><img loading=\"lazy\" decoding=\"async\" aria-describedby=\"caption-attachment-16781\" class=\"size-medium wp-image-16781\" src=\"http:\/\/www.cachc.ca\/wp-content\/uploads\/2017\/03\/Cxy1iBLXgAAx-Xt-1-300x249.jpg\" alt=\"\" width=\"300\" height=\"249\" srcset=\"https:\/\/www.cachc.ca\/wp-content\/uploads\/2017\/03\/Cxy1iBLXgAAx-Xt-1-300x249.jpg 300w, https:\/\/www.cachc.ca\/wp-content\/uploads\/2017\/03\/Cxy1iBLXgAAx-Xt-1-100x83.jpg 100w, https:\/\/www.cachc.ca\/wp-content\/uploads\/2017\/03\/Cxy1iBLXgAAx-Xt-1.jpg 678w\" sizes=\"auto, (max-width: 300px) 100vw, 300px\" \/><p id=\"caption-attachment-16781\" class=\"wp-caption-text\">Dr. Sanjukta Mohanta, dentist at Wellfort Community Health Services&rsquo; Health n&rsquo; Smiles dental care centre<\/p><\/div>\n<p style=\"text-align: justify;\"><span style=\"color: #000000;\">In 2011, WellFort established <a href=\"http:\/\/www.healthnsmiles.ca\/\" target=\"_blank\" rel=\"noopener\">Health n&rsquo; Smiles<\/a>, our CHC&rsquo;s\u00a0dental clinic which provides increased access to dental care to various groups that currently have access to oral health care through targeted government-funded programs. This includes\u00a0children under the age of 18 living in under-resourced families or in the need of urgent care, and qualified seniors over the age of 65. <\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"color: #000000;\">For those who qualify under current funding from the provincial government, services at Health n&rsquo; Smiles include: examinations; x-rays; prevention and oral health education; dental cleanings; sealants; fillings; extractions; partial and full dentures; emergency treatment; and selective root canals. In delivering these services, Health n&rsquo; Smiles staff speak English, Hindi, Punjabi, Gujarati, Urdu, Cantonese, Tagalog, Polish and Arabic &#8211; helping further increase accessibility.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"color: #000000;\">In 2015-16, Health n&rsquo; Smiles provided preventative and treatment services to 3,036 individuals. A total of 6,557 client encounters were made by these individuals during the same year.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"color: #000000;\">While we are very proud of the services we are able to provide\u00a0through Health n&rsquo; Smiles, we also recognize that many more individuals and families are in need, and there remain large gaps in public coverage for necessary oral health care. Therefore, WellFort has also become\u00a0engaged in several oral health advocacy campaigns to challenge the status quo, aiming to improve oral health policy and funding in our region, our province and across Canada.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"color: #000000;\">In the fall of 2016, as\u00a0part of the\u00a0Ontario Oral Health Alliance, we gathered over 250 signatures from local residents and, on November 18, <span style=\"color: #000000;\">2016<\/span><\/span><span style=\"color: #000000;\"><span style=\"color: #ffffff;\">\u00a0<\/span><\/span><span style=\"color: #000000;\">we presented a\u00a0petition to our Member of Provincial Parliament, Jagmeet Singh (Bramalea-Gore-Malton) to table in the Ontario Legislature. The petition called on the Ontario government to:<\/span><\/p>\n<ul>\n<li style=\"text-align: justify;\"><span style=\"color: #000000;\">Ensure that health-care reform includes access to oral health care services for vulnerable people in our communities;<\/span><\/li>\n<li style=\"text-align: justify;\"><span style=\"color: #000000;\">Extend public dental programs for low-income kids to include low-income adults and seniors within the next two years;<\/span><\/li>\n<li style=\"text-align: justify;\"><span style=\"color: #000000;\">Deliver new public dental programs through publicly funded dental clinics to ensure cost efficiency and improved access.<\/span><\/li>\n<\/ul>\n<p style=\"text-align: justify;\"><span style=\"color: #000000;\">Although the Ontario government has promised to expand publicly-funded dental coverage to adults on low incomes, this\u00a0will not take\u00a0effect until 2025. This is too long for many people to wait! WellFort clients and Peel residents like Alisha cannot wait for years, in pain, while their health deteriorates further. <\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"color: #000000;\">As part of the Ontario-wide coalition, WellFort has successfully raised public awareness through <\/span><a href=\"http:\/\/www.bramptonguardian.com\/news-story\/7004642-peel-residents-meeting-with-local-mpp-to-call-for-expanding-public-dental-programs\/\" target=\"_blank\" rel=\"noopener\">local media coverage<\/a>\u00a0<span style=\"color: #000000;\">and we have also used our growing social media presence (Twitter, Facebook, Instagram) to further increase public and political awareness about the impact of\u00a0gaps in oral health care\u00a0in Ontario. We will continue this call for action from\u00a0the provincial government until we have adequate oral health care policy and investment in Ontario.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"color: #000000;\"><img loading=\"lazy\" decoding=\"async\" class=\"aligncenter wp-image-16989\" src=\"http:\/\/www.cachc.ca\/wp-content\/uploads\/2017\/03\/quote-for-chcs-work-2.jpg\" alt=\"\" width=\"550\" height=\"275\" srcset=\"https:\/\/www.cachc.ca\/wp-content\/uploads\/2017\/03\/quote-for-chcs-work-2.jpg 600w, https:\/\/www.cachc.ca\/wp-content\/uploads\/2017\/03\/quote-for-chcs-work-2-300x150.jpg 300w, https:\/\/www.cachc.ca\/wp-content\/uploads\/2017\/03\/quote-for-chcs-work-2-100x50.jpg 100w\" sizes=\"auto, (max-width: 550px) 100vw, 550px\" \/>In addition to our participation in the provincial oral health coalition, WellFort also felt\u00a0that a more structured community mobilization effort in Peel Region was needed.\u00a0<\/span><a href=\"http:\/\/dentaquestfoundation.org\/sites\/default\/files\/uploads\/Compendium_2%2023%2012%20(2).pdf\" target=\"_blank\" rel=\"noopener\">Research<\/a> <span style=\"color: #000000;\">has shown that an<\/span> <a href=\"http:\/\/www.astdd.org\/bestpractices-bpastatecoalitions.pdf\" target=\"_blank\" rel=\"noopener\">effective strategy<\/a> <span style=\"color: #000000;\">to improve access to oral health care at the community level is to build a local oral health coalition to engage residents of the community, community agencies and services providers. <\/span><span style=\"color: #000000;\">We believed that a local coalition\u00a0would better empower our community to advocate healthier public policy and access to necessary services. However, we wanted to make sure that local residents agreed. <\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"color: #000000;\">In the summer of 2016, WellFort hosted a focus group with community members to identify dental needs in the Region of Peel and to gauge interest in implementing\u00a0a local oral health coalition. Focus group participants were selected\u00a0through a convenience sampling method. 23% of focus group participants were between 18-29 years, 55% were between 30-49 years of age, and 22%\u00a0were between 50-65 years.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"color: #000000;\">Participants agreed that there is a strong need for community members to come together more formally around access to oral health care, and in partnership with public and private sectors. They also highlighted many personal and family experiences with\u00a0inequities in oral health and access to oral health care.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"color: #000000;\">Based on research and focus group input, WellFort then took the initiative to establish\u00a0the\u00a0Oral Health Coalition for the Region of Peel. For an initiative of this nature, we recognized the need for strong leadership and shared ownership. A steering committee was established to guide its\u00a0development and implementation.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"color: #000000;\">We\u00a0connected with a number of community partner agencies to actively participate on the committee &#8211; organizations and people who we believed were passionate about ensuring equitable access to oral health care and would be committed to the cause of the coalition.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"color: #000000;\">The coalition is still in its early\u00a0stages, with the\u00a0steering committee holding\u00a0its first meeting in January 2017. In addition to WellFort, coalition members to date include East Mississauga Community Health Centre, Caledon Community Health Services, Peel Public Health, Canadian Academy of Dental Hygiene, and Halton-Peel Dental Hygienist Society. We anticipate that other agencies will join over the weeks and months ahead.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"color: #000000;\">WellFort recognizes that the success of the local coalition\u00a0will be dependent on engagement of policy makers, service providers and community champions.
