CHCs navigate the diverse and complex systems that integrate a range of team-based primary care with health promotion programs, preventative care, community health initiatives and social services to address the social determinants of health to ensure equitable access for individuals, families, and communities across the country. This funding supports initiatives aimed at advancing one or more of the following objectives:<\/p>\n<ul>\n<li>To help CHCs adopt, adapt, scale up, and improve systems navigation efforts to improve client outcomes and grow the CHC knowledge and practice base across Canada.<\/li>\n<li>To help improve overall healthcare access for equity-deserving populations in Canada and overcome barriers to accessing services and continuity of care.<\/li>\n<\/ul>\n<h4><strong>SNAP Project Goals<\/strong><\/h4>\n<ul>\n<li><span style=\"color: #299df7;\"><strong>Enhance accessibility:<\/strong><span
