RESPONSIBILITIES:

• Provide primary health care services to patients of all ages in a CHC model of care including health assessments and diagnosis, health care management and therapeutic interventions, screening, referrals, education, follow-up as well as health promotion and the prevention of illness and injury in accordance with the Standards of Practice for Nurse Practitioners.
• Collaborate with members of an interdisciplinary team to plan, implement, coordinate and evaluate patient care in consultation with patients and their families.
• Participate in the development, delivery and evaluation of educational and health promotion programs within the community as needed.

QUALIFICATIONS:
• Bachelor of Science in Nursing.
• Registered Nurse in the Extended Class (as recognized by CNO)
• Masters in Nursing preferred
• Preference of experience in a community health or primary care setting
• CPR and First Aid
• Knowledge and experience in health promotion, education and program evaluation
• Knowledge of community resources, especially in health and social services
• Ability to work within an interdisciplinary team
• Comprehension of French and English preferred
• Proficient with computers, Microsoft Office, and digital communication tools

WORK CONDITIONS:
• Salary range: $112,573.94 - $129,891.03
