POSITION DESCRIPTION

Full Time, Contract Positions, 35 Hours Per Week

May – August 2026 (Pending funding)

Hourly Rate: $17.60

 

South Riverdale Community Health Centre is a non-profit, multi-service organization that provides primary healthcare, social and community outreach services with an emphasis on health promotion and disease prevention primarily to people of East Toronto. Our mission is to improve the lives of people that face barriers to physical, mental, spiritual and social well-being. We do so by meaningfully engaging our clients and communities, ensuring equitable access to primary health care and delivering quality care through a range of evidence informed programs, services and approaches. As a leader in community health, our locally and internationally recognized community services include cutting-edge primary health services, health promotion, harm reduction, environmental health, community food centre and population-based community programs for marginalized peoples. Pending funding from the Canada Summer Jobs program with Employment and Social Development Canada, SRCHC is looking to fill Health Information Clerk and Community Worker positions, to work onsite.

 

Candidates must qualify under the Canada Summer Jobs program (see criteria below).

 

Health Information Clerk Responsibilities:

  • Work with staff to plan SRCHC’s annual client engagement survey; lead and promote client engagement survey, conduct interviews with clients; data input and analysis of qualitative data
  • Connect with clients via phone, social media, Teams, Zoom, or other virtual platforms
  • Engage in-person with clients accessing services
  • Collection and analysis of qualitative data and reporting
  • Develop presentation on reported findings

 

Community Worker Responsibilities:

  • Assist with community outreach and publicity campaigns including social media, delivery and promotion of summer projects and events
  • Support health promoters with client group activities
  • Assist community groups in organizing and facilitating health and community projects
  • Assist in the development of food access plans and programs, assist in programming focused on food security
  • Develop and update promotional materials including information boards, newsletters and flyers
  • Support the development and updating of program resources and resource lists
  • Assist with client reminder calls, client chart filing and maintenance
  • Provide administrative assistance to staff and programs
  • Assist in the day-to-day activities of the Health Centre

 

Qualifications:

Canada Summer Jobs Requirements:

  • Be between 15 and 30 years of age at the beginning of the employment period
  • Be a Canadian citizen, permanent resident, or person to whom refugee protection has been conferred under the Immigration and Refugee Protection Act for the duration of the employment
  • Have a valid Social Insurance Number at the start of employment and be legally entitled to work in Canada in accordance with relevant provincial or territorial legislation and regulations

Additionally:

  • Post-secondary education preferred, in the field of social work, social service, or health-related studies
  • Strong research, evaluation and analysis skills (Health Information Clerk position)
  • Interest in food security and food access (Community Worker position)
  • Proficiency in the use of computer technology and various software applications
  • Strong verbal and written communication skills
  • Interest in community health and working with marginalized groups
  • Ability to work in a team environment with diverse communities
  • Police clearance documentation will be required for employment
  • Proof of double COVID-19 vaccination status will be required for employment
