About Central Hastings Family Health Team

Central Hastings Family Health Team provides patient-centred, timely access to high-quality health care using a responsive, collaborative, multidisciplinary approach that fosters wellness, disease prevention, health promotion and health education.

About the Role

We are seeking a Primary Health Care Nurse Practitioner (PHCNP) for a permanent 0.8 FTE position to provide holistic, client-centred care in a supportive and community-focused setting. This role will be a partnership between Gateway CHC and Central Hastings Family Health Team located at the Gilmour Medical Clinic. This position requires the Nurse Practitioner to carry their own roster and act as the primary care provider for their assigned clients.

Key Responsibilities

  • Conduct health assessments and physical examinations across the lifespan.
  • Use evidence-informed clinical reasoning to formulate diagnoses.
  • Collaborate with clients and care teams to establish and coordinate care plans.
  • Identify trends in client health needs and contribute to program planning and advocacy.
  • Maintain clear, accurate, and timely documentation (EMR, reports, requisitions).

Qualifications

  • Current registration with the College of Nurses of Ontario as a PHCNP.
  • Member of NPAO with current liability insurance.
  • Five years of community health experience preferred.
  • Completion of “Prescribing Narcotics and Controlled Substances” (NPAO preferred).
  • Proficiency in Microsoft Office and electronic medical record systems.
