| POSITION DESCRIPTION | The Registered Nurse works as a member of an interdisciplinary care team providing episodic care, wound care, basic needs, and service connection referrals. The clinical team is a mobile team that works in various areas in the city with individuals that have experienced barriers accessing care and services. The team has quick access to refer to in-house Social Workers, Outreach Workers, Community Developers, and support staff specializing in the areas of Mental Health, Systems Navigation, Diabetes and Chronic Diseases while out in the community. |
