POSITION DESCRIPTION | Nurse Practitioner (Clinic) Full Time Regular position, 1.0 FTE (35 hours/week), Bargaining Unit – Ontario Nurses Association (ONA) - Region 3, Local 95
Parkdale Queen West Community Health Centre (Parkdale Queen West CHC) is a dynamic, community-based health care organization serving mid-west downtown Toronto. Offering a broad range of services, including primary health care, dental care, harm reduction, health promotion, counselling, and community development programming, we are also committed to serving people who face barriers to health care, and we strive to reduce these barriers. PQWCHC is a multi-site organization.
General Responsibilities:
Reporting to the Manager of Clinical and Allied Health, this position is responsible for providing holistic health care to a full roster of clients, including those who live in our catchment area and those from our priority populations: individuals experiencing homelessness, people who use substances, individuals identifying as LGBTQ, individuals identifying as Indigenous, individuals who have low income, youth, and seniors. In addition to seeing clients in the clinic, the NP may provide primary care services to clients at outreach locations. The NP also acts as a supervisor to health care students and participates in research and quality improvement activities. Our clinical team works closely with our mental health, harm reduction and health promotion teams to ensure cross-team and cross-site collaboration and the NP will work with staff from those teams in providing service to individual clients, as well as in program development and implementation.
Skills & Qualifications:
