Temporary Part Time Position: 14 - 17.5 hours per week

Contract Period: ASAP to March 31st, 2026 (with possible extension)

Salary: $36.36 - $42.90 per hour

Job Summary

The West Toronto Diabetes Education Program (WTDEP) is a partnership of five community health centres in West Toronto and East Mississauga. The main goal of the program is to help the client improve their quality of life of clients 18 years and older living with Type 2 Diabetes and those at risk of developing Type 2 Diabetes.

Working within an interdisciplinary health promotion framework, the Diabetes Prevention and Wellness Specialist will facilitate and evaluate group education classes, provide health information and advice, organize group class registrations, provide team support during outreach events and search out program outreach opportunities.

The incumbent will ensure that all organizational policies are followed and adhere to the overall objectives of the program and the standards of the College.

Primary Responsibilities

  • Organize and facilitate group health education sessions for diverse populations, including the development of culturally appropriate materials and approaches.
  • Conduct community outreach initiatives to raise awareness about diabetes prevention and management and develop a strategy to maximize community engagement.
  • Coordinate activities between healthcare providers, educators, and community organizations to enhance program delivery including but not limited to evaluation.
  • Develop and distribute educational materials on diabetes management, nutrition, and lifestyle modifications.
  • Answer clinical questions before, during and after outreach events from participants.
  • Establish partnerships with local organizations, schools, and community groups to expand program reach.
  • Assist in planning and executing community health events, screenings, and workshops.
  • Provide one-on-one guidance and support to individuals and families affected by diabetes attending events.
  • Maintain accurate records and metrics related to outreach activities, participation rates, program impact, client impact and satisfaction.
  • Monitor and evaluate program effectiveness, making recommendations for improvement.
  • Ensure compliance with all health regulations, policies, and best practices.
  • Develops and maintains effective relationships within LAMP, WTDEP and/or other partner CHC and the surrounding community.
  • Participates in team meetings, LAMP CHC and WTDEP related events.

Required Qualifications

  • Background in public health, health education, nutrition, exercise science, or a related field.
  • Must be a Regulated Health Professional in Ontario (Registered Dietitian, Registered Kinesiologist, Social work etc.)
  • Current membership in good standing from respective professional college.
  • Hold professional liability insurance in accordance with the requirements set out by the respective college in Ontario.
  • Experience working within a community setting.
  • Three to five years’ experience; preferably in community-based diabetes care.
  • Certified Diabetes Educator (CDE) or willing to attain within the first year of employment.
  • Strong knowledge of diabetes pathophysiology, related laboratory values, and pharmacotherapy.
  • Advanced experience and strength in application of motivational interviewing counselling techniques in an adult population.
  • Good working knowledge of the Diabetes Canada Clinical Practice Guidelines.
  • Demonstrated ability to teach and facilitate groups using principles of adult learning.
  • Strong skills in lifestyle counselling (diet, exercise, stress management etc.), evidence-based practice, outreach, health promotion, team work, and problem solving.
  • Superior problem-solving skills and independence, with the ability to prioritize and time manage in a very busy and demanding office environment.
  • Excellent interpersonal skills necessary to work effectively across all levels of the organization’s diverse workforce.
  • Excellent communication skills, both verbal and written, with the ability to clearly convey information and ideas.
  • Positive attitude and dependable with strong initiative and the ability to work both independently and in a team-oriented atmosphere.
  • Open minded, eager, and willing to constantly learn and improve oneself.
  • Respect people of diverse backgrounds and perspectives and foster a culturally safe environment.
  • First Aid and CPR is a requirement and must be kept current.
  • Ability to commute to and from various locations for events and programs. Valid Driver’s License is an asset.
  • Knowledge of LAMP Community Health Centre, East Mississauga and/or the South Etobicoke community.
  • A second language that is reflective of the community being served is an asset.
