| POSITION DESCRIPTION | Job Posting Vibrant Healthcare Alliance Position: Nurse Practitioner Vibrant Healthcare Alliance (“Vibrant”) was formed out of the commitment to create a dynamic and more client-centric approach to enhancing the quality of life and health outcomes of individuals living in the Greater Toronto Area, particularly vulnerable individuals with complex needs including physical disabilities, socially isolated, members of racialized communities and complex older adults. Vibrant has continued the legacy of its parent organizations, Anne Johnston Health Station and Tobias Attendant Care, by providing primary and preventative healthcare and attendant services to the broader community. Our Mission is building healthier communities using integrated services to empower individuals living with complex issues. We are currently seeking a full-time Nurse Practitioner. This is a permanent position with HOOPP and benefits.
