Manager of Palliative Carelink
1.0 FTE
Thunder Bay Site

One of a network of provincial community health centres, the NorWest Community Health Centres is a community-based, non-profit organization funded by Ontario Health, emphasizing access to primary health care and prevention programs and services.

We are currently seeking a 1.0 FTE Manager of Palliative Carelink who will oversee the work of further integrating palliative care services to residents across Northwestern Ontario. The work will include service provision and being a key partner in building the future of palliative care for Northwestern Ontario residents. The focus of this position is twofold: providing front-line leadership and support for the work of Palliative Carelink, as well as an engagement and planning component. Palliative Carelink coordinates access to secondary and tertiary level palliative care expertise and support for providers, clients, and caregivers.

QUALIFICATIONS:

  • Bachelor's Degree in a related field or a combination of educational experiences.
  • Three to five years’ management experience, preferably in a health setting or comparable human service organization.
  • Broad knowledge in the areas of community health, primary health care, and palliative care.
  • Demonstrated ability to be flexible, prioritize, meet deadlines, problem solve, and work independently and as part of a management team.
  • Experience developing and supporting collaborative partnerships.
  • Experience leading a multi-disciplinary team and supporting effective team planning.
  • Understanding and experience working in diverse, low income communities.
  • Knowledge of the community-based health system and of a broad range of health determinants.
  • Excellent organizational, oral, and written communication skills.
  • Skills and experience in supervision, teamwork/team building, and conflict management.
  • Competency with a range of software applications and knowledge of computerized work environments.
  • Awareness of and sensitivity to health issues, particularly within individuals who face barriers to accessing care (under housed, living with current or past substance use, mental health challenges or diagnoses, complex health histories, 2SLGBTQI+, newcomers, non-insured),.
  • A current driver’s license is required, as is access to a vehicle.
