Your information is gathered so that we may occasionally share news, resources and networking opportunities tailored to care providers at Community Health Centres. This list and the information you provide is not shared publicly or with other organizations

  • Networking and knowledge-exchange opportunities are open to care providers at Community Health Centres that are members of the Canadian Association of Community Health Centres (CACHC). Please select your Community Health Centre from the following alphabetical list of current CACHC members..

  • Please select the option that best matches your role at the Community Health Centre. An "other" option is provided at the end of the list.

  • If you selected "other" from the previous list, you may provide further information about your professional designation (optional).

  • Your professional designation may not fully reflect the scope of your role and responsibilities at your Community Health Centre. Please use this field to share any information you wish about your role at the CHC (eg, I spend two days per week as a system navigator for clients; I spend one day per week as Medical Director and participate on management team). You may also list any particular areas of interst in terms of networking and knowledge-exchange with care providers from other CHCs.
